Developmental Screening

Do You Have Questions About Your Child's Development? ​
CCR&R partners with the Southern Illinois Coalition for Children and Families to provide access to a FREE online screening tool that helps families "check in" on the developmental growth of their child.

During the first 5 years of life, your child's brain and body are growing rapidly. Parents or caregivers of children between the ages of birth and 6 years of age living in the southern fifteen counties in Illinois can access  a special  online questionnaire that helps them check on their child's general development. The results help determine if a child's development is on schedule.
What do I need to do?
  • Click the link on the this page. Then complete the  child and family information form
  • Review and accept the information on the parent consent form.
  • Answer a few questions about your child and family
  • Fill out the age appropriate Ages and Stages Questionnaire for your child
What happens after I complete the questionnaire?
  • Results will be sent to you by email at your request
  • Results will also be sent to your child's teacher at your request
